Scantron sheet example


Above is an example of a correctly filled out Scantron Answer sheet. On the left is the student’s name (printed), the course number and date, and the student number written normally. Below that is the student number written digit by digit vertically, with each digit then blackened in the box to the left of the digits of the number. That is the most important step of all.

 

On the right is the version number of the test (A or B) clearly written at the upper right of the page.

 

The main area of the answer sheet is for answers to individual questions. For each answer, blacken the box corresponding to your chosen answer (a, b, c, d, etc.). The blackening must be done with an HB pencil (most pencils are HB), and the mark must be made horizontally, filling the rectangle for the answer chosen, but being careful not to extend into another box.

 

If you change your mind and want to correct an answer, you must erase all marks on the line affected very carefully and thoroughly and then re-insert your answer.
