York University Door Access Card User Acceptance Agreement

This agreement outlines the responsibilities I have as a holder of the York University Door Access Card. My acceptance of this agreement indicates that I have read and understand these responsibilities, and agree to adhere to the policies and procedures established for the York University Door Access Card.
  1. The Door Access Card is intended to facilitate the entry and exit of proximity reader, access controlled labs and rooms on York University premises only.
  2. The Door Access Card is issued in my name as the sole authorized person for access to the appropriate areas of the University. I will not allow any other person to use my Door Access Card. I understand that York University records and maintains data regarding my personal use of my Door Access Card.
  3. Duplication of the Door Access Card or unauthorized use of the Door Access Card including permitting others to gain unauthorized access to labs or rooms using my Door Access Card is considered misappropriation of access privileges. This could result in i) immediate and irrevocable forfeiture of the Door Access Card and/or ii) access privileges to proximity reader, access controlled labs and rooms.
  4. I understand that the Door Access Card is the property of York University and must be surrendered upon leaving York University, whether for graduation, transfer, termination or dismissal. I may also be requested to surrender the Door Access Card for reasons not related to my own personal situation, such as removal of the proximity reader on access controlled labs.
  5. I will maintain the Door Access Card with appropriate security whenever and wherever I use the Door Access Card. I will not tamper or intentionally damage the Door Access Card in any way, shape or form. I will take all reasonable care to prevent the Door Access Card from being damaged, lost, stolen or misused. If the Door Access Card is lost or stolen, I agree to immediately notify Security, Parking and Transportation Services, Campus Services and Business Operations at York University.
  6. I will notify York University Security Services regarding any unauthorized use of the Door Access Card. I will report the discovery of a lost or stolen Door Access Card to York University Security Services.

Conditions of Issue

The card is issued to you as evidence of your need to use proximity reader access controlled labs and rooms. (A proximity reader is a small device located next to the door handle of a door with a light indicator that is typically red. When a card is placed in front of the device, the light will turn green if the card being presented contains appropriate access privileges).

You must not lend your card to anyone to allow them to gain unauthorized access to University facilities.

Rights of the University

The card remains the property of the University at all times.

York University reserves the right to withdraw from an individual, any or all of the facilities of the card and request that the card be surrendered, if evidence is found that the card is being misused in any way. Any unauthorized use will result in confiscation and or possible prosecution.

York University reserves the right to withdraw all cards, suspend some or all of the facilities or replace the card with one or more alternatives, without prior notice.
