Reporting Health & Safety Issues

York University is committed to the prevention of illness and injury through the provision and maintenance of healthy and safe conditions on its premises. It is therefore critical to respond and report promptly to any incident that resulted in, or could have resulted, in injuries, illnesses, damage to health, or facilities.

Occupational Injury

Employees: Incidents, near misses, hazards, occupational illnesses or injuries must be reported to your Management Supervisor, who will then report it to HSEWB. A Workplace Incident Report (WIR) is to be submitted to wir@yorku.ca. For additional information refer to the Incident Investigation Program available on YU Link. Note that you will be required to login using your Passport York information to access this website.

Students and Others: Incidents, hazards, illnesses or injuries are to be reported to your faculty or department, who will then report it to Risk Management. A York University Incident Report (Non-Employee) is to be submitted to Risk Management Services. For additional information refer to the Incident Investigation Program available on YU Link. Note that you will be required to login using your Passport York information to access this website.

Critical injuries must be reported immediately by contacting HSEWB following notification of EMS (911) and Security Services (416-736-5333 or ext. 33333). Access the Critical Injury definition.

Workplace Violence and Harassment

Report any incidents of workplace violence or harassment that you have experienced or witnessed to your Management Supervisor, who will then report it to HSEWB.
