Ontario Health Card

What is it?

An Ontario Health Card proves you are covered under the Ontario Health Insurance Plan (OHIP) and is used to access various healthcare services.

Who Can Get One?

OHIP eligibility relates to your presence and residency in Ontario, and your status in Canada.

Residency

  • Ontario must be your place of primary residence
  • You must have been physically present in Ontario for 153 days in any 12-month period
  • You must have been physically present in Ontario for at least 153 days of the first 183 days immediately after you began living in Ontario

Status

One (1) of the following must describe you:

  • A Canadian Citizen
  • An Indigenous person registered under the Indian Act
  • A Canadian Permanent Resident
  • A Convention Refugee or Protected Person
  • A holder of a valid work permit and are working full-time in Ontario, for an Ontario employer, for at least 6 months
  • A holder of a valid work permit under the Live-In Caregiver Program
  • A holder of a valid work permit under the Seasonal Agricultural Worker Program
  • Have a Temporary Resident Permit
  • A clergy member who will be ministering full time in Ontario for at least 6 months

Note that refugee claimants and Temporary Resident Visa holders are not OHIP eligible.

How to Get One

If you have never registered for OHIP, you must apply to do so in person at a ServiceOntario. You will need to bring four documents:

  1. A completed registration for OHIP form
  2. One original document that proves your OHIP-eligible status
  3. One document that proves your residency in Ontario
  4. One original document that proves your identity
