For new drivers, please attach a Ontario 3-Year Driver Abstract and a copy of their drivers license (front and back) when submitting a completed driver profile form. Incomplete/unattached documents will result in a delay.
A reminder that a new Driver Profile must be submitted upon the renewal of an existing driver’s license.

If flying on or over private property, please have the property owner(s) sign the License Agreement Permission Template. Please attach all completed agreements with your application.

Complete the highlighted fields with the appropriate information before forwarding to the volunteer. The Letter of Understanding should be signed by both parties and be in place before the volunteer arrives in the department.
